Lumina Vision Rehabilitation partners with physicians, optometrists, ophthalmologists, and other healthcare professionals to help patients bridge the gap between diagnosis and daily function.
If you have a patient who has received a diagnosis affecting their vision — macular degeneration, vision changes after stroke or TBI, or age-related decline — but is struggling with daily tasks, home safety, or independence, Lumina may be a fitting referral.
In-home occupational therapy focused specifically on low vision: daily living skills, home safety modifications, adaptive equipment training, fall prevention, and caregiver education — delivered by an OT trained in this specialty.
To refer a patient, call or email directly — Daniela will follow up with you to gather relevant history and coordinate a home visit. A formal referral form is in the works, but isn't required to get started.
